Practice Charter
Our Responsibilities to You
- You will be offered a health check on registering with the Practice.
- We will offer an urgent consultation on the same day but not necessarily with your own doctor.
- We aim to see you within 20 minutes of your booked appointment time.
- You will be given an explanation for any delay of more than 30 minutes.
- We will explain the likely effects of any drugs and revive your long-term medical needs at agreed times
- Emergency care is available 24 hours a day, 365 days of the year.
- We will visit you at home if you are too ill to be brought to the Surgery.
- We will offer you advice on diseases prevention and health promotion.
- You will have access to your medical records, written after 1 November 1991 (subject to any limitations in the law and subject to a charges).
- If you believe that you need a second opinion, we will try to help you obtain this.
- The contents of your medical records will be kept confidential.
- We aim to have repeat prescription requests ready within 48 hours.
- Doctors and staff are involved in continued education and training.
- You have the choice whether or not to participate in research of medical student training.
- Your suggestions and comments on our services will be welcome.
- Any complaints will be dealt with as swiftly as possible. Please contact your Doctor or the Practice Manager.
- You will be received and addressed with courtesy and respect.
- We have the right to remove patients from our list if they repeatedly and persistently ignore their responsibilities to us and other patients.
- The premises are smoke free.
- Copies of our Practice Charters are available to all our patients upon request
- You can speak to one of the Doctors or Nurses on the telephone
- All test results will be avaliable to patients upon request as soon as they are received by the practice.
- You will have a usual GP, you will be seen by that GP unless you wish to see another one.
Your Responsibilities
- We ask that you treat the Doctors and all the Practice Staff with the same courtesy and respect.
- Please do everything you can to keep appointments. Tell us as soon as possible if you cannot.
- You should not expect to be given a prescription every time you visit the Doctor.
- Please remember to check your cupboards before ordering repeat prescriptions, order repeat medication responsibly
- The less time a Doctor spends travelling, the more time is available for patients. Please do not ask for a home visit unless it is strictly necessary. We will always visit in case of genuine need but please be aware that the doctor could see 3-4 other patients in the time it takes to do one home visit.
- If you are unhappy with us or the services we provide, you have the right to leave our list and register with another practice.
